Rotation Sites: Preventing Lipohypertrophy & Complications

Updated 2026-02-01

Summary: Your injection sites are finite "real estate." If you ruin them with scar tissue, you lose the ability to administer effective research. By adopting a strict rotation policy—never injecting within 1 inch of yesterday's spot and rotating through different body "zones"—you keep the tissue healthy, responsive, and capable of absorbing your peptides consistently for the long haul.

This habit seems harmless, but biologically, it is destructive. Repeated trauma to the same cluster of fat cells causes a condition called Lipohypertrophy. This results in rubbery, scar-like lumps under the skin that are not only unsightly but functionally useless. If you inject into these lumps, your peptide will not work. Site rotation is not just about comfort; it is about data integrity.

The Biology of Lipohypertrophy

When you inject, you cause micro-trauma. The body heals this with inflammation. If you re-injure the spot before it heals, the fat cells hypertrophy (get big) and fibrous scar tissue forms.

  • The “Dead Zone” Effect: This scar tissue is poorly vascularized (has few blood vessels).
  • The Consequence: If you inject 200mcg of peptide into a lump, it sits there. Absorption becomes erratic and unpredictable. You might absorb 50% of the dose today and 10% tomorrow. This makes accurate dosing impossible.

The “Checkerboard” Strategy

To prevent this, you need a system. Do not rely on memory. Use the “Checkerboard” visualization on your abdomen.

1\. Map the Zones: Imagine your belly button is the center of a clock.

  • Zone 1: Upper Right (12:00 to 3:00)
  • Zone 2: Lower Right (3:00 to 6:00)
  • Zone 3: Lower Left (6:00 to 9:00)
  • Zone 4: Upper Left (9:00 to 12:00)

2\. The Weekly Rotation:

  • Week 1: Inject only in Zone 1. Even within Zone 1, move 1 inch each day (like moving squares on a checkerboard).
  • Week 2: Move to Zone 2.
  • Week 3: Move to Zone 3.
  • Week 4: Move to Zone 4.

By the time you return to Zone 1, it has had 3 full weeks to heal. This virtually eliminates the risk of scar tissue formation.

Expanding Your Map (Thighs and Flanks)

If you are injecting daily (or multiple times a day), the belly might not be enough.

  • The Love Handles (Flank): The fatty area on the side of the waist is excellent for SubQ. It is less sensitive than the front of the stomach.
  • The Thighs: Use the top-outer part of the thigh. Pinch the skin well here, as there is often more muscle close to the surface.
  • The Arms: The back of the upper arm (tricep area) is good, but hard to reach by yourself.
